Media and Public Relations

NTOCC provides tools and resources to patients, caregivers, healthcare professionals and policy makers who seek to address challenges in meeting the needs of patients transitioning between care settings. Patient care during transitions is often rushed, responsibility is fragmented among multiple providers and communication suffers across care settings. These failures lead to undue burdens on patients and their families and negatively impact patient safety, quality of care, outcomes and health care costs. The focus of NTOCC is to bring together thought leaders and healthcare experts from various settings to address this critical issue, define solutions and develop tools to address the gaps impacting patient care.

Tools and resources developed by NTOCC are made available to everyone in the healthcare industry including providers, payers, and patients. Information and resources are made available to policy makers and media members who seek a better understanding of the Transitions of Care challenge and how to address it effectively.

Improving Transitions of Care with Health Information Technology

NTOCC believes that for Health Information Technology (HIT) to make a difference in transitions of care, the technology must address several critical steps. The components include standardized processes, good communication, required performance measures, established accountability, and strong care coordination. Without addressing each step, the promise of HIT’s affect on overall transition of care improvement will not be realized.

Because there is little guidance on how to use HIT in ways that specifically improve transitions of care, this paper builds upon NTOCC’s overall recommendations for improving transitions of care and the national agenda as it relates to HIT, and identifies problems and considerations as they relate to NTOCC’s overall goals.
