Associate Members

Organizations that wish to support the work of the National Transitions of Care Coalition may apply for Associate Membership. Associate Members help increase awareness of transitions of care challenges and the tools and resources created by the coalition.

There is no fee for becoming an associate member. Organizations participate based on their willingness and ability to advance the coalition’s objectives. Associate Members receive advanced notification of information and the opportunity to comment on coalition activities, initiatives, and tools.

Associate Member Application

There are two types of Associate Members.

Consulting Associate Members are individuals who participate in testing, piloting, and commenting on programs and tools developed by the Coalition.

Industry Associate Members are individuals who are involved in the industry that will promote and utilize the programs and tools to their customers/clients that have been developed by the Coalition.
